Storage of Medical Records

Barwon Plastic Surgery stores medical records in an electronic medical record system, and will only collect information which is necessary to undertake delivery of medical care.

Disclosure of Information to Third Parties

We request in writing your consent to release information regarding your medical care with Barwon Plastic Surgery to a third party. This may include your GP, and other medical specialists. A consent form will be provided when you register as a new patient, to obtain your consent. Allowing us to liaise with other health care practitioners involved in your care, optimises medical care.

Photographs are an important part of medical records. As part of your medical treatment, photographs may be required. Should photographs be required, we will provide you with a consent form to obtain your consent.
